To authorize monthly donations from your bank account, please fill out our monthly donor form and send it to us by mail, email or fax, along with a void cheque.

Changing your information
To change your monthly donation amount or update your credit card or banking information, please contact us at donorcare@stephenlewisfoundation.org, 416-533-9292 or 1-888-203-9990 ext.0. Please note that it may take up to two weeks for your information to be changed in our system.
Tax receipts for monthly donations
If you are making monthly donations through your bank account or credit card, your annual receipt will be sent out in February for the previous tax year.

As a monthly donor, you may change, pause or cancel this arrangement, or revoke your authorization, at any time subject to providing 30 days’ notice to the Stephen Lewis Foundation. To obtain a sample cancellation form, or for more information on your right to cancel a pre-authorized debit agreement, you may contact your financial institution or visit cdnpay.ca. You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this pre-authorized debit agreement.
